Context
The NGO was nearing handover on a solar package at operational health facilities and needed an independent technical eye through the final stages. Earlier projects in the region had been signed off with latent defects that surfaced months later as outages, by which point recourse to the contractor was difficult.
Approach
We supervised installation milestones, ran witnessed commissioning tests, and inspected mechanical, electrical, and protection works against the approved design. A live defects register tracked each item from first observation through contractor response to verified close-out.
Findings
The majority of defects were configuration and protection issues — incorrect inverter settings, missing surge protection, mis-labeled distribution boards — not headline equipment failures. Several would have caused nuisance trips or unsafe conditions during the rainy season.
Recommendations
Documented contractor close-out on every defect before sign-off, a handover dossier covering as-built drawings, settings, spares, and operator briefings, and a 6-month performance review point with the contractor still on call.
